This form is to be printed out for each sample of student work that is sent in for post-moderation

This form is to be printed out and attached to any assessment that you have written yourself that you are sending in for pre-moderation

This form is for teachers, tutors or moderation contacts to fill out and include with any pre or post moderation sent to ATTTO.

The Post Assessment Plan Template can be used to record the end result of the assessment.
This ensures that feedback has been given by the assessor to the trainee and that the pre assessment plan was followed. Both the trainee & assessor must complete parts of this template.
ATTTO requires all Assessors who are completing their Unit standard 4098 assessment to use this template.

The Pre Assessment Plan Template can be used by the assessor during a Pre Assessment meeting to discuss how the assessment will take place.
The template outlines the responsibilities of the assessor and trainee in the assessment process. Both the trainee & assessor must complete parts of this template.
ATTTO requires all Assessors who are completing their Unit standard 4098 assessment to use this template.
